I wasn't sure where to put this to feel free to move it!

I am teaching my 4rth grader to take notes after/during reading of History and science texts. I want him to be able to eventually write an outline of what he reads in his textbooks (maybe not this year yet! but I want to work up to it!) I have been searching for a kid friendly form. Something to help in the transition of learning this. If I have him just write down facts from what he reads he can identify them pretty well - but they are very messy and disorganized on his paper. So, anyone know of a template or from or somtehing that will help?

I hope my question makes sense!

My thing is I want to start him taking notes on his textbooks at 4rth grade. . . so I was hoping someone had a tamplate or something about doing this with the younger set kids!
