I like Jensen's Format Writing for the basic forms. It takes the student through sentence construction, then paragraph, then finally the essay format, with a research paper process at the end. (there's also a business-writing section, which my oldest found quite helpful). It's not exciting, but it is thorough.

The various assignments at every level come with topics, but I usually either assign my own from literature or other coursework, or I let the student choose a topic.

Some details here, in my still-developing weekly plans for my 9th grader. An awful lot of assignments are free-choice with regard to topic; I predict that he will write a good bit about history, science, and current events, because those are his interests. Still, I've tied quite a few assignments for the semester (that's all I've finished so far) to ancient history/literature.

Assignments, by the way, are for different models of paragraphs and essays: definition, process/how-to, cause-and-effect, comparision, and so on.
