Does anyone have their students fill these out to get some feedback for the past year to help with future planning? I love the idea and I know of families that do them with success. My boys on the other hand are not included in that group. Yet, that doesn't mean I won't attempt it again this year! [:D I love these type of things!

How do you get boys, are anyone who would rather not tell you how they feel about stuff to talk? I give my boys the option of filling out the questionnaire or giving me their answers orally. I could use some ideas for questions. I've had them rate different things we did or books on a scale. General questions about what they like best or worst. Anyone have any better ideas?

I just did this for the first time with my older three girls, and it was fun! I love reading their responses and thoughts.

Here are my questions:

:: What was your favorite subject to learn about this year?
:: What skill or subjects do you think you made the most progress in?
:: Look at your "weekly" and list your 4 favorite things to do every week:
1.         &nbs p;         &nbs p;         &nbs p;3.
2.         &nbs p;         &nbs p;         &nbs p;4.
:: How about your 4 LEAST favorite things to do:
1.         &nbs p;         &nbs p;         &nbs p;3.
2.         &nbs p;         &nbs p;         &nbs p;4.
:: What is your favorite part of the day?   Why?
:: What is your favorite day of the week? Why?
:: What were some of your favorite things to do this year? (This can be anything….not just schoolwork.)
:: What did you like about the “block schedule?”
:: What did you NOT like about the “block schedule?”
:: List anything new that you would like to do or learn about next year.
:: Do you have any ideas for changes for next year?
:: Name 2 field trips that you would like to go on.

Suzanne, I like the questions you listed and it gives me some ideas on how to reword some of my questions. Yours feel less overwhelming. I'm looking forward to your feedback from doing it this year!

Neat idea!

Since we lean unschooly, we reevaluate all the time, and would not likely be doing something they disliked for the whole year, but the whole questionnaire sounds fun.

Surprisingly, my boys filled at the questionnaire and did great compared to last year(they pretty much refused to do it last year). I learned some things and they were more honest with me about things that they normally don't say anything about during the school day. I was actually a little surprised on the books and activities they liked and disliked.
