My 3 yr old dd, who has been potty trained for over a year, has started having accidents during the day, sometimes several times a day. She denies any pain that would signify an infection. She is quite nonchalant about it, like it's no big deal. We just clean up and move on. It is getting a bit frustrating.
I tend to treat these things as a phase, but in the back of my mind I worry that there is a physical cause. At what point, or sign, do you take a child in for this?

My 7yr old dd has a similar history. At age 5 we had her checked out at the University Clinic for Nephrology because our pediatrician couldn't see 1 kidney on the sonogram. It is true that Maria only has one kidney, but it is very healthy and not a reason for concern. It is also not the reason for the wetting. At the clinic they told me that for little girls of that age it is very common to be day wetting. They just don't like to interrupt what they are doing/playing to go potty...and then it is too late. Until recently I had to remind Maria regularly to go to the bathroom...and sometimes even force her to go. Sometimes she would even lie to me and say she had gone when it wasn't true! It takes an extra effort from us mommies to teach them to go regularly, but with time they do learn 
Molly, at this point I wouldn't worry about it. Just help her to go at certain intervals throughout the day. Your dd probably just enjoys her play so much, that getting up to go is just too burdensome or she just forgets about it.

Not to digress too much, but my experience with a child not wanting to take the time to stop and visit the bathroom. My oldest dd at age 3: I suggested that she might want to try and use the potty. Her response: "But I just did that yesterday."

Is it a full wetting or is it just a leak. We had a leaky problem after pottying with one of our dd's. She had a labial adhesion.

I was thinking too that is she preoccupied with somthing and just a little reminder and offering to go with her .Its maybe a big job for a little one and just needs some help right now in that area .
My little girl who is now 21  peed on a puzzle once and destroyed it .She was so into making the puzzle she forgot to get up to go pee .
I find boys way more difficult I still have to remind ds6 when out and about .I have to often bend right down to his level and ask him close up " do you have to go " he is just so oblivious at times when with others or out and about . If you can maybe try and take note of the time span in which her accidents are happening .Then you can try to jump in maybe the 1/2 hr or so before and remind her to go to the potty .
Also another htought is does she have younger siblings ? a baby ? a baby who is demanding ? maybe this is an attention thing .If so the potty reminder help would be giving her you rtime and posative attention anyway  she might really just appreciate mummy taking the time for her
Also remember she is 3 yrs and may live to be 94yo there is no real rush when we see the bigger picture

Oh, she is peeing buckets when she goes, no leaking here! Usually, she makes it into the bathroom and just doesn't get her pants down in time. Yesterday, she went while riding her little bike outside. She doesn't care at all, in fact I have to make her change because she would just keep going.
I have been through this with others. I guess I just need to pay more attention and remind her now and then. Certainly easier than mopping floors and extra laundry! Hey, I know I should be thankful that she is potty trained and even dry at night!! That is a big accomplishment around here.
I think it is my nurse's training, which is all very foggy these days, that plants the thoughts in my head that there is a medical problem behind every change in behavior.

Nancy, I love your digression, "But I did that yesterday"! Sounds like something my little one would say too.
