I just switched ob's today.....to one 5 hours away from us. I am 5 wks pg and am already having trouble with my glucose and have had 2 miscarriages in the past 18 months. I asked my local ob yesterday to check my thyroid and progesterone in hopes that I could do everything possible to protect this pregnancy. I also asked what I can do about my high glucose numbers (esp. my fasting one). He said that because I have had 6 successful births that he would not do either test. He did say he would check my HcG levels to make sure they are rising properly. He also said there is nothing I can do about my fasting numbers. He also would not see me until I am 10 or 11 weeks along. He just seems so uninterested in helping me with this pregnancy and also uneducated as far as dealing with diabetes in pregnancy! There really aren't any other choices here besides him.

I got disgusted and started calling/emailing both the Pope Paul VI and the Teyeyac Clinic to get second opinions. They both said there were possibilities of things we could do. So I talked it over with dh and called and made an app't with Tepeyac. We do have lots of family up in that area and I would probably have a scheduled c section (was planning that anyway). Dh will be working a LOT in DC in the next few months and then prob. one week a month through the end of the school year. Sooo, we could make it work sort of okay..not sure how app'ts at the end of pregnancy would work...

but, is this crazy and irresponsible to be this far away from my ob?

I'm very happy to be able to get into Tepeyac and be with a practice that will support our family and do everything to help me protect this pregnancy...I feel so blessed. Even today before I called, dh told me that he needed to be in DC on Thurs and Friday so a Thurs. app't would be best. I told him I doubted they'd get me in that fast or be able to accomodate us so exactly and when I asked for the app't the first thing the receptionist said was "we have an opening on Thurs." (!!!)

I just don't know though whether I am making a risky decision though....

well, in a true emergency you'd go to the ER first anyway.. many things can be handled over the phone.. and I at least have no problem traveling that far or further while pregnant until i'm close ot my due date anyway.. so I'm not sure why it would be a risky decision.

You can get lots of info on the diabetic diet online so you can start that. And I also know that exercise will help your body deal with the glucose.

yeah Jodie, I was thinking the same thing about the ER in case of emergency.

I am on a diabetic diet (that I was put on with my other pregnancies by the diabetic nurse educator) and am also exercising already after each main meal- 30 minutes of walking. If I don't walk my numbers are near 200...walking keeps them down close to 140 and below. I just can't skip the walking and if it is that hard now when it is really NOT pregnancy related, I'm worried about how much harder it will be to manage once the placenta kicks in. (I was dealing with high numbers before I got pg and was trying to lose weight to get them in check so was already doing the diet and walking thank goodness!)

Thanks for helping me think this through and for the suggestions! I'm also up for any suggestions on trying to fix my fasting numbers!

Hollee, with high numbers already at the beginning of pregnancy, I think Tepeyac will probably refer you to an endocrinologist (as that's what I had to do). But for referral and insurance, you'll need a GTT. Keep a track of your fasting numbers to support the data.

Have you had this high blood sugar problem before?

Well I will have one relieved friend/daughter's godmama if I get a referral to an endo. She is an RN/professor and has been telling me to get a referral from my doc since before my last m/c. My doc did not seem to feel it was that bad and told me there is nothing to be done about the high fasting numbers.

I have been journaling all of it -what I eat, whether I exercise or not, my numbers...etc.

I was diagnosed with GD during my pg with #6 and was able to really do a good job controlling it with diet and exercise. I went on glyburide the last week or two because my fasting numbers were getting a wee bit high....98-103 or so (if I remember correctly).

Then when I got pg with #7 (m/c ) I started checking my numbers just for cautiousness and they were a bit high but not too bad....then with #8 (another M/C ) I did the same and they were kind of all over the place-always a bit high. But I have always walked after meals to try to control them as much as I could (and was strict about the diabetic diet).

It just seems that with each one, they kept getting a bit worse and I have been checking my fasting a couple times a week since Easter and it has been high then (but I was not as concerned about it as I was not pg and was hoping that diet and weight loss would bring it back down)

My numbers now are the highest they have been yet and I am having trouble balancing the numbers with not spilling ketones (if I go a bit lower on my meals to help the numbers stay down, then I spill mod-large ketones) I am getting different numbers from the same exact foods too which is really weird. I eat the same thing for breakfast and this morning after eating and walking, my 1 hr was 154...it's all a confusing mess to me!

Did you end up on insulin? Starting when?

We discussed at length in this threadabout blood sugar and pregnancy

You need to check your 2 hour, and your fasting. Fasting should 95 or under, and 2 hour 120 or under.

I get gestational diabetes as soon as I get pregnant. My body doesn't cooperate with the placenta very well. I go on insulin, the first full term pregnancy at 20 weeks, the second full term pregnancy at 12 weeks. My GTT was very high the first time, and this time, although my GTT results were very normal, I knew it wasn't right (I was taking the numbers in the waiting room). I think the test results got switched (poor lady who got diagnosed with GD from my tests!). I kept my fasting numbers and was seen almost immediately when they just kept getting higher.

It's just been my experience when you're way out of the norm (norm: elevated levels around the 28 weeks) at Tepeyac, they send you to a specialist.

Make an appointment with the OB ASAP (esp since you're risk of M/C) and call the nurse to see what needs to be done with your situation.

I have problems with this as well, and agree that it would be great if you could see a specialist. They just take it all a lot more seriously. My specialist told me regular OB what to do, and he did it. He may not have believed in it, but he did it! If I hadn't gotten to the specialist, I probably never would have had my last ds.

I don't think it's crazy at all.
In fact, it sounds like you'd be nuts NOT to get a better provider.
Also, a note, fluctuating sugars can be caused by more than gest d. tyroid and other things can cause it or excerbate it.

A full work up sounds like it's in order, it sounds like you know it and want it done, so it makes perfect sense to me that you'd choose to go out of your way to get a dr that is as considered for your health as you are.

Thank you all so very much for the reassurance! I do feel that it is the right thing (although after way more trouble than usual trying to get all my dc to pack tonight for just a 2 day trip I may question myself again! )

I guess my ob here just has no clue about diabetes and stuff along those lines...I WAS checking my bg 2 hrs after meals until he scoffed at it and told me that my numbers at 1 hr were way more beneficial and those were the ones he wanted to know, not the 2 hr ones. So as of today I am back to 2 hrs.

Can't wait til my app't tomorrow and am just hoping that all is well with the little one so far!
