I really like the one from the Grade 2 Lesson plans by CHC, if you can manage to find a copy in time, the whole Appendix for religion that year has been amazing. Even the RCIA children's program has been pleased with dd's knowledge.

Beth, I have an old booklet "Confession Prayers for Children" by Rev. Daniel Dougherty. I typed it up with plans on making it printable, but haven't formatted it yet. But I'd be happy to send you the Word .doc file if you PM your email address.
It is short, but very sweet and helpful for a young child.
Here's a little excerpt:

I Think of My Sins
(These questions may help you to remember your sins.)
Did I say God’s holy Name in a rough way?
Did I say words which were not right for me to say?
Did I stay away from Mass on a Sunday or a holy day, when I could have gone?
Did I talk back to my father, my mother, my teacher, older people?
Did I do what they told me do?
Did I think, say, or do anything to be ashamed of? Alone, with others?
Did I steal anything, or harm what some one else owns?
Did I copy in school?
Did I waste my time? Was I lazy?
Did I go with bad girls or boys?
Did I tell lies, or say wrong things about anybody?
(If you think the sin is a big sin, you must tell the number of times you did it.)
If you do not know how to tell a sin, you can say to the priest, "Father, I have a sin I do not know how to tell."
He will help you.
If you think of something else, put it in your own words,
If you have no sins to tell, say to the priest, "Father, I have no sins since my last confession, but I am sorry for ---------------." Tell him a sin from your past life.

I Think of Saint Peter
Saint Peter was a very good friend of Jesus.
Jesus made Peter the leader of Apostles.
He wanted Peter to do big things for Him.
He trusted Peter and Peter knew it.
Often, Peter boasted that he would never leave Jesus.
Our Lord told Peter not to be too sure of himself.
The night before Jesus died, Peter ran away, afraid that something might happen to him.
Three times that night, when some people said that Peter was a friend of Jesus, Peter said,
"I am not. I do not know the Man!"
Poor Peter! Afterwards, he was very much ashamed of all this.
He cried very much. He was very sorry.
When Jesus rose from the dead, He met Peter.
Everyone thought Jesus would say hard things to Peter.
Instead, Jesus asked him three times, "Peter do you love Me?"
Peter was very much ashamed, but very happy.
Jesus is always kind, when we are really sorry.
When I am, I act as if I were no friend of Jesus.
I run away from Him.
Now, I am sorry.
Jesus asks me, "Do you love Me?"
I say, "yes, Jesus, I do love You."
"I will try not to sin any more."
