I can't remember where I got it, but we sometimes used this list of questions in the evenings:

Did I honor and obey my parents today?
Did I get along well with others?
Did I use my time wisely?
Did I guard my tongue? (Was I careful with my words?)
Did I guard my heart? (Did I have a good attitude? Did I send away ugly thoughts?)

My mother always used this format:

How have I offended God today in my thoughts?

How have I offended God today in my words?

How have I offended God today in my actions?

How have I offended God today by omission (not doing what I was supposed to be doing)?

She would pause after each question, to let us examine, and sometimes give small examples for the little ones.
