At the dentist yesterday, they asked us if we have well water (we do). She thinks that's why my kids are starting to have cavities when they haven't, previously (city water). She recommended a fluoride rinse. Everything I've ever read about fluoride is pretty negative, but I'm not excited about having problems with cavities, either.

Is there anything else I can do besides a fluoride rinse?

It is better than taking it internally.
As long as they don't swallow any and don't eat anything for 30 minutes after using it, I have heard the rinses are good ideas. We use them and Xylitol-type rinses. Especially for my cavity prone dd. Xylitol in am and fluoride at night.
There is a thread somewhere in here about flouride. If I knew how I'd link you--but I don't. Someday I'll have time to learn how to do it all here.

We drink fluoride free water - but have recently started fluoride toothpaste to avoid the cavity problem.

I've read both sides of the debate - we have a friend who is a dentist and recently led the effort to fluoridate the water in our neighboring town. We don't have fluoride in our well-water, but use fluoride toothpaste. Here is a link to the anti-fluoridation campaign: Fluoride Action Network.

Like what others have said, I think that there is evidence that fluoride works topically but not any that shows taking it internally is beneficial (and may be harmful). We don't do any fluoride (we filter city water), but I would consider using it topically if it were an issue.

Xylitol is supposed to help and you can also get that in gum form.
